How is a trade scored?
Each side is valued at the live TCGplayer market price of every card on it. The two totals are compared; a trade within 10% of the larger side is called fair, and anything wider names the side that comes out ahead.
Does it account for condition?
Values are for ungraded cards in near-mint condition. Adjust your expectations for played or graded copies — a PSA 10 can be worth many times the raw market value, and a played card far less.
Should I balance the trade with cash?
That's exactly what the delta figure is for: if the sides are far apart you can add cash or swap a card to close the gap. Re-run the numbers after any change.
